These forms are generic only. Contest Organisers can choose to use these forms or make their own. It is recommended that only the Header, Dates & Fees, on the forms, are changed by Contest Organisers. If the Contest Organiser is the QBA, then these forms will probably be used. In all other cases, contact the Contest Organisers for the correct form.

These lists do not apply to State & District Competitions. Please check the By-Laws for the current definition of a Stage March for State & District competitions. At April 2008, the definition is: " Music other than a march played in quick-time on parade."
